«Vorskla» — «Kudrivka» — 0:1 (3:4 on penalties)

Goal: Litovchenko (9)

«Metalist 1925» — «Left Bank» — 1:0

Goal: Kalitvintsev (90+1)

Thus, «Kudrivka» and «Metalist 1925» will play in UPL next season.
